Custom Buttons: Which dimensions to use?

If the button image is not perfectly square, the button is not rendered well. sometimes there's flipping and some times, the buttons change its dimension. Does the plugin support only square-like button images or is there something am not doing right?

What's wrong with the rendering exactly?

1. The images tend to take the same size irrespective of the different sizes.
2. In some instances it is rendered upside down. I have to invert the images to get the desired look.
3. Seems the title bar size affects the rendering too.
