It is unclear whether a person doesn't use any drugs or whether they just didn't take time to look into that section and respond appropriately.
Please make it more explicit.
First approach might be:
persons who use it should tick "use"
those who don't - tick "don't use"
But it is also lame.
Some people use it quite much and quite often whereas some others use a bit once in a while. Which is a significant difference, but both groups would mark themselves similarly.
Then, those who don't use. There are those who use so little and so rarely that they count it as "don't use" meaning "I don't use it in day-to-day life". Whereas there are those who really don't ever use any. Again, significant difference but same mark.
Then, there are those who use but are trying to quit. Or those who don't but are interested to try.
As a special category, there are medicines that are similar in effect to drugs.
Some people use them as drugs for enjoyment or relief, whereas others use them for treatment of some other condition but are bound to suffer their side-effects.
