Lefthand column titles & subtitles

As there are a number of issues in each of the areas of the screen layout I will group them in that way for the moment, so this issue will be about Lefthand column menu & buttons

FAIR section titles:

• remove 'P1 - ' to 'Pn - '.
• perhaps a colour background will help indicate these titles are not questions.
• Check accessibility guidelines for the best options unless we have very different user-selectable stylesheets for accessibility.

Question titles:

• remove 'C1 - ' to 'Cn - '

• It should be possible for the DOT file creator to relate these questions to specific FAIR principles e.g 'F1.1' or have 'Q1 - ' to Qn - ' in the DOT file

• When selecting questions towards the bottom it is not obvious that the question is displayed on a normal sized window & I would assume similar on a mobile device. It should take the learner to the next question in the same fashion clicking the submit button does for those questions not visible at the top of the open window.

Submit Assessment button:

• Should possibly be in the central column.
• currently has no functionality apart from taking the learner to the top of the screen if the question is not visible in the open window.
