In some cases, fragment uploads succeed but are reported as a failure, resulting in a duplicate upload. The best way to fix this would probably be to scan for a valid upload before attempting another upload, and if a valid upload is found, ignore the so-called failure.

This issue appears to be fixed by 1897cfb. This issue will be reopened if necessary.
