I recently added a second point to a MULTIPOINT geometry using the edit form for the document. After saving, the feature appeared as a single point between the two pairs of coordinates I had specified despite the screen distance between the two locations being significant. Zooming in/out did not help. Deleting the geometry attachments and allowing them to regenerate did not help.

I was able to reproduce this problem. At first glance, it seems to be associated with the clustering algorithm. In the example I played with, turning off clustering solves this issue. Is it the same for you?
