Create a reusable form field widget

Create a reusable form field widget that is going to be used for forms on the hamsa lomi app. The current style we have is that there will be a header and a rounded form input. In the focused state, the form field shows the green color outline:

This is more of a refactoring and enhancement task. Make sure you update all the current form fields that are currently created in the app.
