Create FORM with two inputs fields with user email and phone number and submit button

• when a user enters a not valid email or phone should add error message under the input field,
• in case if the user enters a valid email and phone number and clicked the submit button, it should open a modal with some message,
• when clicked outside of Modal, close the modal.
