Embedded Form Builder

As a convention HR Associate
I need a Form builder embedded into FoxDen
So that I can collect applicant data effectively

Acceptance Criteria:

1. Form Questions are customizable
2. Form allows for the following answer formats:

• Radio Button
• Short Text
• Long Text
• Checkbox

3. Form can support conditional logic
4. Data from the form can be extracted into .xls or google sheets if necessary

Some additional field types I plan on adding:

• Checkboxes
• Multiple select dropdown (think any of Workday's dropdowns if you've used that product)
• Yes/No (shows two radio buttons with yes/no labels. Just a shorthand way of implementing those kinds of questions)
• Agreement (shows a checkbox that says they agree to terms presented in the question)
• Signature (like JotForm)
• File Upload (with filters for what types of files are allowed and min/max of files)
• WYSIWYG (for additional text formatting)
• Date (with minimum and maximum values)

Also, I think the long text field should have an option to allow formatting via Markdown
