Why are the data of some biomarkers are completely outside the reference range in labevents.csv?

When I analyzing the data from labevents.csv I noticed several cases similar to the following.
The heamoglobin levels are well below the reference range in almost all the tests
The C-Reactive protein levels are always above the reference range.
I just wanted to clarify whether these variations are due to presence of some deseases or can there be any other reasons associated with recording the test values?
