How to interpret a plot where the reference path and sample path are the same? #4

Could you help understand the plots where the reference path and sample path are the same?

Here's the example of such plot,

This looks to visualize this event. In IGV, I see a small fraction reads evidencing the break but not understanding the AD of "32,31" and how it matches up with the plot.

In addition, I wonder how the reference blocks B, C, D are determined because those blocks don't seem to play a role in this event.
