Includes updates to MySQL translations for table creation. Disables adding specific collation to character fields (we are already using NVARCHAR and will rely on the DB default). Disables appending of ON PRIMARY to statements. Attempts to detect presence of a PRIMARY KEY, and if not found will attempt to upgrade another index to CLUSTERED as required by Azure.
