How does it work? Does it use Firefox sync?
Do I need to press the sync button every time if I want to keep it updated, or does it do it automatically after I press it for the first time?

Yes, it uses Firefox Sync.
You need to press the button every time. In my experience, Firefox sync is sometimes finicky. There are occasions where it will sometimes claim that it synced, but it didn't actually. If sync were automatic when that happens, it could accidentally delete notes!
