No instrument content reflected in designer mode

We have created content for an instrument in designer mode. We save an push to github.

No content appears in the instrument

content is not reflected in data collection mode

Workaround by deleting instruments from termforms folder

The content reflects when I test with your data. I tested with different data too, it works. Perhaps, you haven't saved the instrument or may not have added it to a plan? @vkozloff
