Use probot to automate creation of GitHub App

We can use a GitHub App manifest to make creating a new GitHub app for FlowEHR deployments almost automated for new environments. The caveat is we would need to use a small webserver (i.e. Fast) to get GH's redirect response to then retrieve the config details.
