Referral editing link on patient detail page

The link to the admin panel entry corresponding to a referral followup request on the patient detail page is broken. It currently directs to an action item editing page.

The issue is in action-item-blurb-active.html where admin_url is defined for the patient detail page. This variable need to change values depending on if an "action item" is an actual Action Item object or a Referral FollowupRequest.
