Health Summary - Vitals and Symptom Survey Chart Layout

Problem

Patients wanted a cleaner way to view all of their vitals and a health summary layout that better explained what information is primarily for the patient and what information is more for their care team.

Solution

Change the Layout and locations of the Symptom Survey chart and the vitals charts to reflect the following:

1. Remove Vitals Summary from the front page
2. Move the Symptom Survey table to the second page
3. Remove the header with patient information from the second page
4. Change the order of the symptom scores from most recent to oldest
5. Add a "Details for your care team" header to the top of the second page
