v1.1: Modal Dialog

This update includes the following contents.

• add a modal dialog to adjust TOC which you can open in navi-bar button

At the same time, remove the menu to adjust toc in menu bar.

v1.0: All features work even in Edit mode

• move by clicking an item in TOC
• add toc-menu buttons in navi-bar - toggle-expand / back-to-top / go-to-bottom
