The HTML form will have a textbox for entering courseID and checkboxes for files, pages, modules, syllabus, assignments, discussions. Put a button for "go" below this.
Have 'Enter Canvas course ID' as a placeholder inside the textbox and 'Select the type of content you want to ingest' above the checkboxes.

Show the below message as an instruction above the input fields -

Please ensure that you have added the UIUC Chatbot as a student to your course on Canvas before you begin ingesting the course content. The bot email address is [email protected] and the bot name is UIUC Course AI.

Maybe use Checkbox.Group for that: https://mantine.dev/core/checkbox/#checkboxgroup

We'll also have to figure out where to put it without making things too confusing for users.
