Approval system to publish a page for CC and AC

Currently there is only a single level of approval which corresponds to having gone to the orientation. However, there are really two levels of approval needed in the system for users who can publish content on the site. They need to have gone to the orientation and been approved in their role by the service org.

Describe the solution you'd like

• Remove existing admin interface for approving users
• Update filament interface (Add an interface to see user roles and approval for each role #2273) to support both orientation and role approval
• Update application to look at both orientation and approval when determining if a user can publish content
• Update copy on the site as needed

Describe alternatives you've considered

Another alternative was explored as part of #1670 but would likely take longer and was making that page more complex. See: #1670 (comment)
