Showing wrong Heading on the Subject Registration

Describe the bug
Showing wrong subject name when we go to registration via the work-list rule.

To Reproduce
Steps to reproduce the behavior:

1. Create 2 subject types
2. Add worklist rule on completing one subject registration go to another subject registration
3. See the Header of registration screen

Expected behavior

• Correct subject type name should be displayed in the header.
• Make sure works in all flows like encounter type forms
