[Visit Scheduling] Display only relevant values in the dropdown

AC:

• Do not show voided concepts in the 'Value of' dropdown. Currently it shows. Also in other places within the rule where show dropdown as well - voided values should not display.
  

• In Answer concept dropdown show all possible answers as a dropdown instead of the need to type

• We are showing some non-applicable answer concepts as well. Show only those applicable for the selected concept.

• Should work the above way in all rules where this 'quick add condition' drop-downs are there.

Some edge cases to consider:

• Concepts could be voided but still in use (form element not voided)
  -- even if this is taken care of, it might still make sense to define rules based on voided concepts to handle based on historical data.
• there is an 'active' flag on concepts - should that be considered instead of voided?
