Same concept added twice in the same question group

We can able to add the same concept name in the same question group and this is not handled in the system.

Env : Staging
org : dashseven
user : dinesh@dashseven

Steps to reproduce :

1.Login to the webapp
2.Click on the app designer
3.Click on the encounter,program or subject type
4.Create the question group
5.Add the same concept in that question group

Expected Result : Same concept cannot be added or duplicates are not allowed
Actual Result : We are not getting are any error
