Problem

We would like to allow users to add their conference details in a beautiful form.

Proposed solution

Add a form which allows users to:

select the conference from a drop-down
enter the code
add their name
add their home address (the form should do a geo lookup to find the lat/long.
add a map of the resulting lat/long, which users can adjust.
add a departure date.
optionally add a gravitar, otherwise, add a random custom picture
