Add uniqueness validation to AppointmentHold/AppointmentLoan #1725

We have an issue where someone can submit the appointment edit form multiple times and end up with duplicate holds/loans on the appointment. Here's an example in production: https://app.chicagotoollibrary.org/admin/appointments/16920

We should at least add a validation to prevent this, and ideally a database constraint as well.
