consistent numbering in LaTeX/PDF #2

Some of the numbering in the PDF version is inconsistent with the new html version,
and in all cases I noticed, it would be better to change the numbering in the PDF version.

For example: Figure 3.12 and equation (4.1)

This involves the numberwithin command in the header.
