The wiki says "you need to create a new folder in the mods directory of you minecraft installation" but never says what to name the folder. That info should be on the wiki (probably within the quoted sentence).

The folder can be called as you like, within reason. This is a replacement archive fashion. The ability to create a mod using Custom Stuff 4 only on the Forge 2655 version. The mod itself can work on the latest versions.
