Actual Results

When enrolling a patient in these programs only two fields are displayed: Program and admission date. A state field is not available.

Expected results

Three data entry fields should be made availble. That is program, state (with the following options; Active on program, Dropped from program, Patient died, Started drugs, Transferred out to a health facility) and admission date.

Steps to reproduce

Login to eSaude POC->registration->search for a patient->enroll a patient to the CCU program or mobile clinic program

Use test server, on the dev server, one is not able to enroll a patient to a program.
