Restrict patient to a single initial form per program enrollment

Patient is automatically enrolled in the MH program when first initial form is saved. Another initial form cannot be saved until the patient is disenrolled from previous program.

Might be able to use the HFE tag or equivalent method, if looking up program enrollment is easy to do that way.
