Unclear how to use an "instance" sfdcType #292

Is it correct to assume that "instance" sfdcTypes allow users to login to custom instances of salesforce (as opposed production/dev (login) or sandboxes (test)).

If so, it's unclear how to supply that custom instance.

Perhaps this issue should be modified to state that I'm unable to use a custom domain with this plugin. The enums for logintype are only login & test. would like to be able to e.g. supply a custom subdomain.
