CH Core Patient - citizenship binding extensible as required (Oliver Egger, ahdis)

With CH ELM we have an issue that we need to provide Kosovo as a country code, however the CH Core rule prevent this, because the value needs to to be in the country code list, and there are some country codes which are not in this list, see ahdis/ch-elm#135 for more detials

Solution: replace ISO lists by the official country list of the Federal Statistical Office:

2. instead of using a constraint we should use the published BFS ValueSet as a binding
