Reduce number of checkboxes

Instead of three checkboxes, the signup screen should show a single checkbox so users can agree to all terms, policies, and guidelines. The checkbox should be on the same screen with the username/password form.

We'll need input from @abhas on what screen "Learn More" should take users to.
