This is because the add-on comes pre-setup for google's various websites, but requires that you manually add the other search engines that this add-on is supposed to work on.
To do so, perform a search on DDG. Then click the extension's icon in your toolbar and look for text that says "Activate".
If this text is not visible, first open the extension via the browser's extension page and then scroll down to the many google sites that are already listed. Just above the huge list is the toggle slider to automatically enable on those sites. Toggle that to off, which will turn it off on all of the various google sites.
Then go back to your search page, refresh, click the extension, and the "Activate" option should be visible.
It's rather weird to me to say that it works on various search engines and then it just focuses so heavily on google. But this work around should get it going for you. However once you do this, it'll then remove the sites that you select to block.
You may need to refresh the page again after activating the extension in order for the "block this site" text to appear below each search result.

Naturally, you'll want to toggle the "automatically enable" slider back to on after you finish doing this.
@EzlinRye Thanks! I couldn't figure out how to enable it for DDG. I once did it on my other machine, but completely forgot. Unfortunately there is no info about this on extension main page :(
Expected Behavior

Search results in DuckDuckGo do not show the block link. Block links are visible when using Google in the same browser
