RESKC-950:PD Printing: Approved&Submitted does not support Create PDF

for PHS398_CoverPageSupplement_2_0-V2.0 > Create PDF -no data provided
in form.
Approved and Submitted Proposals are not populating data in the PHS398
Cover Page Supplement 2-0 FORM when users attempt to create the PDF.
(Create PDF) is printing a blank form and populates with no transmitted
data. 
S2S Opportunity Search > Forms > Select > Create PDF for the PHS Cover
Page Supplement form.
The system will generate the FORM, but none of the proposal or
questionnaire data appear in the PDF post-submission.
To reproduce:
Kuali Demo 1: 
Search and open proposal number 72 (approved submitted PA-C-R01)
Go to S2S Opportunity Search screen > Forms tabl
Click "select" PHS398 Cover Page Supplement 2-0
Click "Create PDF"
Results:
Generates a PDF form stylesheet, but NO DATA answers appear.
Desired Results:
KC needs to print the PHS398_CoverPageSupplement_2_0-V2.0 form for user
on Approved and Submitted proposals with transmitted data populated.
MIT KC Production PD 29529 - 
Change/Corrected proposal, Created from copy of KC Native (not Migrated
from Coeus) proposals.
Approved and Submitted status, submitted S2S 10/14/2018
Confirmed in eRA Commons that the Grant Image included correct data for
PHS398_CoverPageSupplement_2_0-V2.0
