Inconsistency between codec and metadata_indexing

It seems like metadata_indexing_{include,exclude} may not work depending on the codec.

Specifically, metadata_indexing_include = ["foo"] is always re-written to exclude metadata.foo, 1 but the Flat codec will write metadata["foo"] to foo 2.
