Bug: Quote naming is flaky #2

The quote naming form behaves improperly, though behaviour seems to vary. The two misbehaviours I've witnessed are:

The dialogue seems to function but will not save your entry on the first try.
The dialogue flickers open then closes immediately (noticing this exclusively on recent beta Firefoxes).
