Inferno requires a R4 sample to pass Inferno test #4739

Describe the bug
A clear and concise description of what the bug is.
provides a sample for STU3, however, the FHIR server also supports R4 and we need a 4 compliant, otherwise Inferno will not pass.

Data provider
CosmosDB

To Reproduce
Steps to reproduce the behavior:

Start Info
Run Single Patient test (Step 5)
Fails

Expected behavior
Should pass the inferno test using the sample data
