Updated node-fastcgi

Hello! I'm the developer of node-fastcgi. Just wanted to let you know that I integrated some of the changes in your fork, and my original version should be usable as it is now.
It now relies on the REQUEST_URI param to build the request url instead of SCRIPT_NAME and PATH_INFO. The REQUEST_URI variable is not in the CGI standard, but it is provided by all major HTTP servers (configurable on nginx).
