Current version is 1.2, adapted from OTN form.

Working on this over on https://github.com/mhpob/latitudes/tree/act if you'd like to assign me.

@jdpye some questions:

  • Do you know if any of the metadata submittal forms were created under any sort of license? It should be reflected here.
  • Is OTN the copyright holder?
  • Do you know who the original authors were?
