Comments on a document typically fall into categories - it is a bad idea to lump them all into "comments". I'd suggest aiming for a more refined notion where comments are categorised into the "type" comment. Concretely I suggest you add a "tag" or "type" label to each comment, and comments get listed by type. e.g. "proposed re-wording", "question/answer", "request for clarification". I suggest having a hashtag for to categorise each type of comment (and restrict it to one tag).

One note on this. There might even be a benefit in creating a more abstract View that allows the user to pick how they wish the comments to be displayed. Timeline view versus Categorical (Grouped) view, etc.
