Now that Ax supports fixed_features in the Service API (see facebook/Ax#2015, facebook/Ax#2372), we no longer need a CustomAxClient class.

Also, Models.GPKG has been deprecated and had to be replaced with the newer Models.BOTORCH_MODULAR (facebook/Ax#2427).

Using Ax 0.4.0 also means that we need to drop support for Python 3.9.
