Custom form missing

We've installed the most recent version of this plugin on an otherwise brand new OSTicket installation (v 1.9.14) and followed the installation instructions from the wiki and YouTube video (https://www.youtube.com/watch?v=y0wL6213TH0).

Everything seems to work well, but as instructed by the video, when I go to Help Topics -> Custom Form, the form from Equipment Plugin doesn't show up. Any ideas as to what might cause this?

We're really excited about and appreciative of this plugin, we will be able to donate to development in the future if we can get it working for our shop : ).

After re-installing the plugin, it seems to be working now. Not sure what was broken before, maybe some permissions were wrong on some files. Ideally it would do a sanity check when activating/using the plugin and display some type of warning.
