Minor update of reset behavior

The is a fixed reset behavior of the pemicro probe under PyOCD.

Clean up the release 1.0.0

In compare of release v1.0.0.post1, there is just cleaned up the leftovers (all in comments) from J-Link PyOCD plugin template.

• Removed the hard-coded version number in setup.py so the git-based version will be used.

• Exported the plugin as a pyocd.probe entry point. This makes it possible for pyocd to find pyocd_pemicro once the package is installed.
• Ensured the package can be released as a universal wheel.
