'Accept' button on 'Submission Request' not working on 1.6.1

The 'Accept' button does nothing when entering the 'submission Request' page. The only way to get notified records into the system is to click the green 'tick' button when logging into the system.

Clicking on 'Delete' immediately after clicking 'Accept' seems to cause major issues as well. After clicking delete, I ended up with a record that could not be selected. Every time I tried, the system would stall, and eventually give up with a dialog 'Error'.
