Simplify the feedback mechanism

Current problem

  1. Require code to create new feedback form
  2. Require to create 1 form per session

Ideal

  1. No code required if we want to collect more feedback.
  2. 1 form to rule them all. Maybe we can just disambiguate using the feedback submission date.
