Reporting: License report new structure: cover page

The cloned document from issue eclipse-sw360#376 shall have a new structure with four kinds of elements:

• fixed text, part of the template
• information that should be fixed, but can be overridden by the user (sw360 has default texts for this)
• information that can be queried from the sw360 database
• information that can be queried from the license xml files

Proposed document structure
