Error Occurred When Settings Excel Sheet Is Missing

Error occurred when uploading a ODK form without settings excel sheet.

We may consider:

1. Handle the error by providing a temporary form_title and form_id
2. Throw error with a meaningful error message, to advise user to add settings excel sheet in ODK form
