If myapp.herokuapp.com is mapped to myapp.customdomain.com - then whitelisting works only when the app is accessed via myapp.herokuapp.com
i.e myapp.customdomain.com is allowed via any IP not listed in the white list.

This is due to this line which specifically checks for herokuapp.com
