There was a problem finding data.
eeprom =

NIC is: XL710-QDA2 (0x1583)

I don't understand how can I find phy0_offset

Any help will be apreciated.

Hi.
Have same problem with my XL710-QDA2.
lspci output:

Problem resolved by uploading Intel firmware into QSFP+ module. No card manipulations was needed.

I have XL710-QDA1 [8086:1584], and Finisar QSFP-40G-SR4 modiles.
