Create a backbone view to handle adding/editing addresses (shown in a dialog).

Template should include:

Type (address/phone/address + phone)
- This would show/hide fields depending on what the option selected is
Address Type (home address/work address)
Primary Address (yes/no)
Streetline 1 (text box)
Streetline 2 (text box)
city, state/province, zip code, country (textbox with geonnames typeahead)
phone1 (text box)
phone2 (text box)
fax (text box)
