Multiple Input - Environmental factors

Allow users to 'add' extra rows by clicking on a + icon besides or underneath the i icon for a sub-domain. eg. in Products and Technology enter 5 values one for each different product.

On clicking the + icon, a dialog box should appear having a title "Additional " and below that 5 rows with the similar view as that of the main row(-4 to 4 with similar colored checkboxes and a comment field) and an OK and Cancel button.
